Insurance Participation

Children's Clinic
participates with most major insurance companies. Co-payments are payable at the
time of service. Because plans vary, submission to your particular insurance
company depends upon your plan.
HMO Participation

Children's Clinic
participates with many HMO's. Please call if you have a question regarding your
HMO. Co-payments are payable at the time of service. Because we participate with
many HMO's, a 72-hour notice for referrals is requested.
Payment Policy

Payment for office services
is due at the time of your office visit. If a portion of your visit is covered
by insurance other than the above-mentioned plans, we will provide you with the
information necessary to fill out your insurance forms.
A doctor's
signature is not required.
If you need
